Hillwood Training offer Incident Investigation Services using the TapRooT® Investigation and Root Cause Analysis system. Experienced personnel can facilitate an investigation to find the root causes and recommend corrective actions for injuries, accidents, equipment failure, near misses or system failure.  We can assist proactively in improvement projects and audits in order to stop an incident/failure before it occurs, improve productivity or solve a difficult process issue.

TapRooT® is the RCA tool of choice for many multi-national companies & govt departments including, Rio Tinto, Exxon Mobile, ConocoPhillips, Woodside Petroleum, Santos, Ensign, Qantas, Australia Defence Forces and numerous hospitals, utilities and councils.

There are many instances where it is advantageous to seek external assistance from a qualified independent third-party expert such as when:

  • It is likely an incident will lead to legal proceedings
